Apgar score interpretation
The Apgar score adds up five parameters, each scored 0–2, for a total between 0 and 10: Appearance (color), Pulse (heart rate), Grimace (reflex irritability), Activity (muscle tone), Respiration. How to read it: 7–10 is normal, 4–6 calls for moderate concern (the baby may need stimulation or oxygen), and 0–3 is severe (resuscitation required). The score is taken at 1 and 5 minutes, and again at 10 minutes if the 5-minute figure comes in below 7. Example: color 2 + HR 2 + reflex 1 + tone 2 + respiration 2 = 9 (normal).
Clinical context
The anesthesiologist Virginia Apgar devised the score in 1952, and it went on to standardize how newborns are assessed around the world. A low Apgar at one minute shows up in plenty of healthy babies and mostly reflects conditions in the womb. A low 5-minute Apgar, though, warrants investigation with cord blood gases and neurological observation. On its own the score does not predict long-term neurological development. You'll see it used in delivery rooms, in neonatology, as a hospital quality indicator, and in the civil registry. It isn't a substitute for the Neonatal Resuscitation Program (NRP) algorithm, which is what actually guides intervention in that first minute of life.
FAQ
Does a low Apgar mean brain damage? No. Most babies who score low go on to develop normally. Cerebral palsy lines up much more closely with prolonged depression and cord blood acidosis.
Why score at 1 and 5 minutes? The 1-minute reading captures intrauterine status and how the baby adapts right away. The 5-minute reading shows how the baby responded to interventions, and that one carries more weight prognostically.
Is Apgar valid for preterm infants? Yes, but read it with caution. Tone and reflexes are naturally lower in preterm babies, which pulls the score down even when nothing is wrong.
Should I delay resuscitation until the 1-minute Apgar? Never. You follow the NRP algorithm right away; the Apgar simply documents how the baby responded.
