RMR vs BMR: what the difference actually means

BMR (Basal Metabolic Rate) comes from a tightly controlled measurement: a 12-hour fast, full rest, a thermoneutral room, awake but not moving. RMR (Resting Metabolic Rate) is taken under more ordinary resting conditions, usually after a light breakfast and a brief rest. The two end up close, and in practice RMR ≈ BMR + 10%. So a BMR of 1,500 kcal points to an RMR near 1,650 kcal/day. Equations such as Mifflin-St Jeor and Harris-Benedict estimate BMR on paper, but for day-to-day meal planning the number gets treated as RMR, since nobody is measuring patients at strict basal conditions. The gap is small, and it rarely tips a clinical decision one way or the other.

Applications

In clinical nutrition it sets a caloric baseline that you then multiply by an activity factor (PAL) to reach TDEE. For body recomposition protocols, knowing the BMR/RMR floor keeps you from cutting too hard. When precision really counts, like bariatric pre-op, oncology or the ICU, indirect calorimetry is still the gold standard; the equations are the quick, free stand-in for everything else.

Which one should I use to cut weight? Either works. Put the activity multiplier on top, then take off 15-25% for a deficit you can actually stick with.

Why do equations and a metabolic cart disagree? Equations are built on population averages, so any one person can land ±10-15% off, and that's most pronounced in lean athletes and obese patients.

Does muscle mass change RMR a lot? Not as much as gym folklore claims. Each kg of lean mass adds only about 13 kcal/day at rest.
