Three bodies agree on the same advice here: the World Health Organization (WHO), the Brazilian Ministry of Health (MS) and the Sociedade Brasileira de Pediatria (SBP). They call for exclusive breastfeeding for the first 6 months, then complementary breastfeeding up to 2 years or beyond. To get the months still left, the tool uses remaining = max(0, 24 − current_age_months). The exclusive window works the same way, with exclusive_remaining = max(0, 6 − current_age_months).

The guidance traces back to the WHO/UNICEF Global Strategy for Infant and Young Child Feeding (2003), which Brazil adopted through the Estratégia Nacional para Promoção do Aleitamento Materno (ENPACS). What does breast milk actually do? It passes on immunity through IgA antibodies and cuts neonatal mortality. It lowers the chance of otitis, diarrhea and lower respiratory infections. It deepens the bond between mother and baby, and over the years it feeds into cognitive development. Mothers who can't nurse directly have support too: the Banco de Leite Humano network run by BLH-IFF/Fiocruz is the largest in the world.

Applications

Pediatricians, lactation consultants (IBCLC), nutritionists and the family-health teams of the Programa Saúde da Família (PSF) all reach for numbers like these. They use them to counsel mothers, to time the slow introduction of complementary foods after 6 months, and to fit a return to work around maternity leave. In Brazil the Lei 11.770/2008 (Programa Empresa Cidadã) stretches that leave to 180 days, which leaves more room for exclusive breastfeeding.

FAQ

Can I offer water during exclusive breastfeeding? No. Breast milk is roughly ≈88% water and covers a baby's hydration on its own through the first 6 months, even when it's hot out. Adding water only thins out what the baby takes in and opens the door to infection.

What if I cannot produce enough milk? A lot of what feels like low supply turns around once the latch is fixed and feeds happen often enough (8–12 times/24h). Before you reach for formula, talk to a Banco de Leite Humano or an IBCLC consultant.
