Peak respiratory rate: reference values and clinical interpretation
Respiratory rate (RR) is the number of breaths per minute, and in PALS/ACLS protocols it sits next to SpO2, heart rate and blood pressure as a core vital sign. A healthy adult at rest breathes 12-20 rpm. Infants run faster (lactants 30-60 rpm). Push the body to peak exertion and a sustainable rate lands somewhere around 40-60 rpm. Example: take a 30-year-old whose RR holds at 28 rpm while resting. That counts as tachypnea (>24 rpm), and it's a clinical red flag that can point to sepsis, pulmonary embolism or pneumonia. Ergospirometry measures RR through a mask wired to a gas analyzer, and it remains the gold standard for exercise testing.

You'll see it in routine nursing assessment and ICU monitoring, in sports conditioning for endurance athletes, during polysomnography (PSG) to screen for sleep apnea, and across pulmonary rehabilitation programs built for COPD and asthma patients.

How to count RR correctly? Watch the chest rise for a full 60 seconds and don't tell the patient you're doing it, because the moment they know, the rate shifts. One full inhale plus exhale equals one cycle.
What is tachypnea? An adult resting RR above 24 rpm. Drop below 12 rpm and you're looking at bradypnea, which shows up in opioid overdose and neurological depression.
Why is RR so different in children? Their lungs are smaller and their metabolism runs hotter. A newborn hitting 60 rpm is perfectly normal, and the rate eases down little by little as the child grows toward adolescence.
