Dental Anesthetic Dosage Calculator
Enter a patient's body weight and this tool estimates how many anesthetic cartridges fall within a safe range. In Brazilian dentistry the workhorse solution is 2% lidocaine hydrochloride (sold as Xylestesin, Alphacaine or Lidostesin) mixed with epinephrine for vasoconstriction. The usual ratio is 1:100,000 for routine work, dropping to 1:200,000 when the patient has a cardiac condition. A standard cartridge holds 1.8 mL, which works out to 36 mg of lidocaine (1.8 mL × 20 mg/mL).
For a healthy adult, the recommended maximum of lidocaine with vasoconstrictor sits at 7 mg/kg, never going above an absolute ceiling of 500 mg. A 70 kg patient lands at roughly 490 mg, which is around 13 cartridges on paper. In practice most sessions stay well under that, usually 4 to 6 cartridges. Children and older patients need the dose scaled down.
Applications
Dentists, endodontists, oral surgeons and dental students reach for this during treatment planning, and it earns its keep on the longer appointments: a mouthful of restorations, third molar extractions, full-mouth rehabilitation. Keeping the total in check is what guards against systemic toxicity from a lidocaine overdose, which can show up as CNS excitation, seizures or cardiac arrhythmia. It follows ANVISA RDC 78/2007 on injectable anesthetics.
FAQ
Can I use this for children? No. Pediatric dosing runs on age-specific, weight-based protocols (usually 4.4–7 mg/kg with vasoconstrictor) and should only happen under specialist supervision.
What about patients with heart disease? Switch to 2% lidocaine with epinephrine 1:200,000 and cap the session at 2 cartridges. If the patient is unstable, check with their cardiologist first.
Does this replace clinical judgment? No. Treat it as an educational reference. Before giving any local anesthetic, work through the patient's medical history, ASA classification and current medications.
