The survival "rule of 3" puts the figure at ~21 days without food if water is still available, though how long you really last comes down to how much body fat you carry. Glycogen runs out in roughly 24h, after which the body shifts over to burning fat and enters ketosis around day 3 to 5. Gandhi's hunger strikes ran as long as 21 days. The documented medical record is 382 days (Angus Barbieri, 1965, a case of severe obesity under supervision). Lean people start eating into muscle mass after about 7 to 10 days.

The biggest danger once someone is rescued is refeeding syndrome. Bringing food back too fast after 5 or more days of fasting can trigger fatal hypophosphatemia and arrhythmias. That's why NICE guidelines say to start at 10 kcal/kg/day.

FAQ

Why does body fat matter? Each kilogram of fat holds about 7700 kcal. An adult at 20% body fat is sitting on a reserve of more than 100,000 kcal, which can last over 50 days at rest.

Is fasting safe? For most healthy adults, short fasts under 72h are fine. Anything longer should be done under medical supervision, and people with diabetes or who are pregnant should not attempt it.

What's worse, no food or no water? No water, and it isn't close. Dehydration kills within days, whereas going without food takes weeks.
