Perimenopause Duration and Hormonal Transition
Perimenopause (you may also hear it called premenopause or the menopausal transition) is the phase that comes before menopause itself. It usually starts somewhere between ages 40 and 50 and runs, on average, 4 to 10 years until the final menstrual period. What's happening underneath is a steady depletion of ovarian follicles, falling estradiol production, and a compensatory climb in follicle-stimulating hormone (FSH). On the diagnostic side, FSH above 25 IU/L measured twice at least 4 weeks apart points toward the perimenopausal transition.
The clinical signs are fairly recognizable: menstrual cycles that turn irregular (first shorter, then drawn out with skipped periods), vasomotor symptoms like hot flashes and night sweats, disrupted sleep, mood swings, vaginal dryness, and the early start of faster bone density loss (around 1–2% per year), which over time pushes up osteoporosis risk. The Sociedade Brasileira de Endocrinologia e Metabologia (SBEM) and FEBRASGO back menopausal hormone therapy (MHT) for symptomatic women within 10 years of menopause or before age 60, provided there are no contraindications such as a history of breast cancer, thromboembolic disease, or active liver disease.
Applications
Gynecologists, endocrinologists, and primary care physicians lean on this kind of estimate to anticipate when symptoms might begin, schedule bone density screening (DEXA), talk through contraception (which is still needed until 12 months without a period), and tailor hormone therapy decisions to each patient. For women themselves, it gives a sense of the usual timeline and a head start on lifestyle changes such as calcium 1200 mg/day, vitamin D 800–1000 IU, and resistance exercise.
FAQ
How is perimenopause confirmed? Mostly on clinical grounds, using age and the pattern of irregular cycles. Hormonal testing (FSH, estradiol) swings too much from day to day to be reliable, so doctors tend to save it for atypical cases or when premature ovarian insufficiency is suspected.
Is hormone replacement therapy safe? For eligible women who begin it inside the “window of opportunity” (within 10 years of menopause and under age 60), HRT works well and is regarded as safe for easing vasomotor symptoms and protecting bone, according to SBEM/FEBRASGO/NAMS guidelines.
Does this estimate substitute medical evaluation? No. See a gynecologist or endocrinologist for an assessment that fits your situation, particularly if symptoms are severe or they start before age 40, which can signal premature ovarian insufficiency.
