Average Menopause Age & Duration

Menopause means menstrual cycles have stopped for good. You only confirm it looking back, once a woman has gone 12 consecutive months without a period and there's no other physiological or pathological explanation. Worldwide, natural menopause arrives around 51 years on average, and most women fall somewhere in the 45–55 range.

When it happens before 40 it's called premature, or premature ovarian insufficiency (POI); after 55 it counts as late. The run-up to it, perimenopause, tends to stretch over 4–8 years. During that window estradiol swings up and down while FSH climbs. The symptoms most women report are vasomotor episodes such as hot flashes and night sweats, along with urogenital atrophy, lower libido, broken sleep and shifting moods.

Applications

Having a rough idea of how long the symptomatic window lasts makes it easier to plan ahead. It informs decisions about hormone replacement therapy (HRT) with estrogen + progesterone, sets the timing for bone densitometry follow-ups, and flags cardiovascular risk, which climbs quickly once estrogen drops away. Both the Brazilian SBEM and the international NAMS guidelines back individualized HRT for patients who have no contraindications.

How is menopause confirmed? Usually on clinical grounds, once a woman of the expected age has gone 12 consecutive months without a period. Lab work (FSH > 30 mIU/mL, low estradiol) backs up the diagnosis but isn't always needed.

Is HRT safe? For most healthy women who start it within 10 years of menopause and before age 60, the benefits tend to outweigh the risks. It's off the table for anyone with a history of breast cancer, a thromboembolism, or active liver disease.

Why does cardiovascular risk rise? Estrogen helps protect the blood vessels. When it falls off, LDL cholesterol and arterial stiffness usually go up, and that pushes coronary disease risk higher.
